Terms of Release

I/We agree or give consent for the person named in the account creation form to participate at CareFest which is sponsored by Wheaton Bible Church.

In the even that I/he/she is injured while working with Wheaton Bible Church and its representatives and requires the attention of a doctor, I hereby accept full responsibility for any reasonable medical treatment as deemed necessary by a licensed physician.

I/We further agree to hold the licensed physician, the medical facility, the Wheaton Bible Church and it representatives free and harmless of any claims, demands or suits for damages arising from the authorizations and provision of such medical treatment.

I/We understand the nature of the event and do hereby release the Wheaton Bible Church and its representatives from any liability due to accident or injury incurred by myself or child.

I/We give Wheaton Bible Church the absolute right and permission to use my/our photograph(s) in its promotional materials and publicity efforts. I/We understand that the photograph(s) may be used in a publication, print ad, direct-mail piece, electronic media or other form of promotion. I/We release WBC, the photographer, employees, agents and designees from liability for any violation of any personal or proprietary right I/we may have in connection with such use.
